NHS GPs generally just test TSH (thyroid stimulating hormone) as it is inexpensive, but this does not tell us what is going on with actual thyroid hormone levels, also TSH lags behind changes in these thyroid hormones and can be affected by other hormone imbalances in the body.

Diagnosis needs to be made by testing Free T3 and free T4 alongside TSH and by testing for antibodies which determines whether it is Hashimoto’s disease or just hypothyroidism.

However, diagnosis cannot be made just on the numbers, if there are any abnormalities in the full set of TFTs and symptoms are present it is worth discussing a future management plan with the patient and consider starting them on treatment.

Blood Tests

A sample of your blood will be taken within your assessment and sent to our laboratory for analysis. Your blood will be tested for TSH, FT3, FT4, thyroid antibodies, Vitamin D, Vitamin B12, Iron and folate.

Report of findings & Treatment Plan

Having analysed the data from the assessment and blood tests your specialist will explain the diagnosis to you, and discuss the range of treatment options that are available.

Your doctor will design a treatment plan that is specific to your needs. If you require medication, a prescription will be issued.

Progress Review

Throughout your course of treatment, the doctor will monitor your progress through regular consultation and re-testing of the TSH, FT3 and FT4 in your blood. The results will be discussed with you and we'll decide next steps together.

Typically progress reviews are undertaken at the following milestones:

  • 1 month
  • 3 months
  • 6 months
  • 12 months

If we're supporting you throughout pregnancy, then monthly appointments and tests are needed up to 30 weeks. After this it should be taken at least once in the following 12 weeks. Following birth, it is important that thyroid function is monitored carefully, and bloods and review needs to happen at most 4 weeks after birth.
